WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a Family Medicine Nurse Practitioner to meet the growing demand for primary care services at Franciscan Physician Network Winfield Health Center located in Crown Point, Indiana. Experience is preferred. Our family medicine specialists see patients of all ages, from birth to seniors. Our providers diagnose and treat medical illnesses, provide preventive care such as annual checkups and health risk assessments, and provide guidance on maintaining a healthy lifestyle. In addition, they manage chronic conditions and can refer to specialists or coordinate hospital care as needed.

WHAT WE NEED:

Candidate ideally has experience, would thrive in a fast-paced environment and be team oriented. This provider will need to work collaboratively with colleagues and support staff to coordinate and deliver the highest quality of patient care. Candidate will also have a desire to develop strong ties with patients and the community in which they serve. They must display a professional skillset and be willing to build long-lasting relationships with patients and their families.

WHAT YOU CAN EXPECT:

• Practice Hours: Monday - Thursday, 7:00 a.m. - 7:00 p.m.; Friday, 7:00 a.m. - 5:00 p.m.
• Flexible full-time schedule; no weekends or holidays
• Schedule includes one evening per week until 6:00 p.m.
• Average outpatient volume: 18 - 21 patients per day
• Telehealth offered
• Onsite lab and radiology
• Strong mentorship opportunities from seasoned providers

Qualifications:

• Master's Degree Required - and
• Basic Life Support Program (BLS) - American Heart Association - Required
• Registered Nurse (RN) - Required - and
