Job Description

Performs medical and psychosocial evaluation of patient with chronic conditions, end of life and comfort care; extensive involvement in discharge planning and continuity of care efforts; and support and collaboration with primary medical and nursing staff responsible for the patient.

Qualifications

Education & Training:Nurse Practitioner with appropriate Maryland licensure and credentialing to practice under the supervision of a physician in the state of Maryland. ANCC or Palliative & Hospice Medicine certification is preferred.

Work Orientation & Experience: A minimum of five (5) years of direct patient care experience in internal medicine or palliative care, hospice. Experience is appropriate to the management of chronic disease, end of life, comfort care and goals of care.

Additional information

Palliative care nurse practitioner should be available for on site consultations in both Bel Air and Havre de Grace or Aberdeen.
