Job Summary and Qualifications

Under the direct supervision of the Director and according to hospital policies and procedures, the Enterstomal Therapy/Wound Care Nurse provides direct care for people with fecal and urinary diversions and wound care management. Acting within the scope of nursing practice, the person in this position will consult with other members of the health care team in the management of acute and chronic wounds, and will be actively involved in the management and prevention of pressure ulcers. Staff education and policy management is also an integral part of the position.

GENERAL RESPONSIBILITIES

Utilizes the nursing process, initiates assessment, plans the care of patients with ostomy, draining wounds, fistulas, pressure sores, and incontinence.

Assesses all new ostomy patients for knowledge level/deficit skill level and self-care potential. Performs stoma sitting, initiate's ostomy and other wound care areas in plans of care; assures adequate discharge planning and follow-up care.

Assists nursing staff with decision making in clinical situations related to wound and stoma care.

Works with nursing staff and physicians to develop and equipment for patients with ostomy, draining wounds, fistulas, pressure sores, and incontinence.

Evaluates, selects and recommends supplies and equipment for patients with ostomy, draining wounds, fistulas, pressure sores, and incontinence.

Evaluates and approves patients for appropriate pressure reducing mattresses in a timely and cost-effective manner.

Develops and monitors standards of care and assists in the formulation and evaluation of procedure, protocols, and policies related to his/her area of expertise.

Plans, develops, and/or coordinates education for patients, families, and nursing students.

Identifies educational needs of patients and/or families concerning alterations in health, the disease processes the hospital plan of care or discharge plan as they relate to wound/ostomy care.

Orients new nursing personnel and selected nursing students to the role of Enterstomal/Wound Care Nurse.

Provides in-service educational programs regularly to nurses, physicians, and other health care members.

Engages in research projects when appropriate.

Keeps current in his/her area of expertise by participating in professional programs and organizations.

Serves as consultant to patients, families, nursing staff, physicians and other health care team members.

Attends patient care conferences and staff meeting as appropriate health care team members.

Develops a participatory relationship with the Product Evaluation committee to assure availability of products necessary to wound/Enterstomal care.

Maintains current policies for Enterstomal and wound care reflected issues.

Co-chairs and actively participates in skin care task force.

Other duties as assigned.

Experience

Experience in Wound Care - preferred

Education

Required: Graduate from an accredited nursing program

Preferred: Bachelor's or higher Degree; Previous clinical experience

Licenses and Certifications

Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, Texas license must be obtained within 90 days of hire

Basic Life Support (BLS) certification per St. David's Healthcare policy

Preferred: Credential as Certified Wound Specialist

    Preferred: ACLS Certification as per SDH policy
